Minoxidil (Regaine) & Finasteride results after 10 years

Not all HRBR patients require hair transplant surgery in order to treat their hair loss.  We treat many patients with the FDA approved medical treatments for male pattern baldness; Minoxidil and Finasteride. Each patient’s exact treatment plan is devised during a consultation with one of our expert hair loss doctors. We recently received a testimonial from a patient who our doctors have been treating at HRBR since 2005.  He has followed our treatment program of Minoxidil (Regaine), Nizoral shampoo and Finasteride for over 10 years.  He has asked us to share his experience.

Who is suitable for this type of treatment?

In general, Finasteride is believed to be more effective in the 18-41 age group, but exceptions exist and significant benefits from these medications are frequently seen outside this age group. What is Finasteride?

Finasteride is an oral prescription medication which blocks the conversion of testosterone to dihydrotestosterone (DHT), the form of the hormone that causes male pattern baldness. It is marketed in the US as Propecia but this is not available in Ireland. Here patients often use a generic form of the medication called Fintrid (or Proscar) 5mg which can be cut to the recommended dosage.

Finasteride inhibits the action of the type II 5-alpha reductase enzyme that is present in higher concentration in and around the hair follicles of balding men with androgenetic alopecia. After two years, results showed that 83% of the men taking Finasteride either kept their hair or grew more. Seventeen percent continued to lose hair while taking the medication. What Brought this Patient to HRBR?

This patient, like many men and women experiencing hair loss, turned to the internet for information and in his own words he “got lost in the mixture of truth, half-truth, science and complete nonsense” that he found online. He subsequently ended up at a well-advertised clinic that had a famous sportsperson as its spokesman. Here is what he had to say about that experience.

My experience here immediately focused on me considering a menu of shampoos, laser combs and hair systems (modern toupees) and little was done to explain what was happening me medically or how I even felt about losing my hair. There was certainly no mention of possible preventative treatment that might manage the issue. The climax of the experience was being left alone in a room to watch a carefully constructed video that aimed at manipulating my insecurity about hair loss and I assume lead me to sign up to one of their treatments.

Is Finasteride a Permanent Solution for Hair Loss?

Many patients ask what the results of taking Finasteride years later will be. The medical treatments can be effective in slowing down the hair loss process, however, for most men the effectiveness of the treatment will wear off over time. In our experience, most men will start to see the effectiveness wear off 6 – 8 years after starting to take the medications. It really does depend on the patient. The major benefit of Finasteride seems to be in its ability to slow down or stop hair loss, or regrow hair in parts of the scalp that are thin. The long-term ability of Finasteride to maintain one’s hair is unknown. It is usually necessary to commit to taking Finasteride for at least 12 months in order to assess the benefit for each individual patient. In this patient’s case after six months he began to notice an improvement. It is also important to know that Finasteride stops working when a patient stops taking the medication.

Currently, the only permanent solution for treating male pattern baldness is hair transplant surgery. This is an option that a lot of our patients will look at once they begin to see the effects of the medications starting to wear off.

Bryan Robson’s Hair Transplant at HRBR

Why Bryan Robson had a Hair Transplant

Bryan first decided to take steps to deal with his hair loss as a result of some photos taken during a trophy presentation ceremony at Old Trafford.  The angle of some of the photos, taken from behind and at a slightly elevated angle, revealed that the hair loss on his crown was more extensive than he had perceived it to be.

Hair transplant 6-months post op

When a patient walks out of the clinic after their hair transplant surgery, it is far from the last contact he or she has with our team at HRBR. As part of the post-operative care programme, patients are seen by the team on the day immediately after the procedure and again 7-10 days following this. We also schedule both a 6-month and 12-month check-up to keep track of progress. These appointments are all included in the cost of a patient’s hair transplant at HRBR. So, what results should a typical patient at HRBR expect from their hair transplant 6-months post op?

At HRBR we are careful to ensure patients understand that a hair transplant does not produce a dramatic overnight effect. The hairs contained in the transplanted grafts will fall out three to four weeks after surgery. The all-important hair follicle remains anchored beneath the scalp and will begin to grow a new hair about three to four months after the transplant. This subtle regrowth blends seamlessly with existing hair and continues to grow forever. On that basis, a hair transplant 6-months post op should start to display some noticeable benefits. The full effect of the hair transplant will be seen between a year and 18-months post-surgery.

We followed one of our patients, Mark Berry, through his experience of hair transplant surgery at HRBR in a series of video blogs. This video shows Mark 6-months post op at his scheduled check-up. This is a great opportunity for Dr Maurice Collins, Mark’s hair transplant surgeon to assess his progress and for Mark to ask any questions.  At the 6-month post op check-up the surgeon will check the donor area.  At this stage, if all has gone well, the scar should be virtually invisible.

At the 6-month post-operative check-up those images will be taken once again for comparative purposes.  Seeing the before and after pictures is usually a highlight for our patients, as was the case with Mark. This process will be repeated at the 12-month check-up to reveal the result of the hair transplant.

Scarring Following Hair Transplant Surgery

Many patients when considering hair transplant surgery are interested to know if they will be left with scarring following the procedure. The answer to this question is simply yes. A scar is your body’s way of healing following an injury.

Be wary of any clinics offering “scar-less surgery” as this is not possible.

How noticeable the scar is following the procedure is down to a number of factors including the technique used to remove the hair from your donor area, the length of your hair following the procedure, your own body’s tendency to scar and most importantly the skill of the surgeon carrying out your procedure.

FUT or FUE and Hair Length

There are currently two methods of removing the hair from the donor area around the back and sides of your scalp. Follicular Unit Transplantation “FUT” or strip method and Follicular Unit Extraction “FUE”.

FUT Hair Transplant Scars

An FUT hair transplant involves a small section of hair bearing skin being removed from the donor area, this is then dissected down to its individual hair grafts to then be transplanted into the areas of thinning or baldness. When carried out by a skilled surgeon, most patients will be left with a fine pale linear scar in the donor region following this technique. This scar is normally completely concealed with approximately 2-3 centimetres of length in the patient’s hair in this area.

FUE Hair Transplant Scars

With the FUE hair transplant technique each individual hair graft is removed one by one from the donor area. Patients are therefore left with small circular scars where the hair has been removed. In this method the amount of scarring is greater however as it is spread out over a larger area it is often less noticeable and patients can trim their hair shorter in the donor area following the FUE procedure.

Scarring in Future Hair Transplant Surgery

In the FUT procedure should a patient return for further transplantation in years to come a skilled surgeon can remove the original linear scar and thus the surgeon will leave the patient with only one linear scar. In the FUE technique this is not possible and a scar is created for each graft taken.

As the incisions in the recipient (thinning) areas, where the hair is placed, are very small, any scarring in this area should be completely undetectable to the naked eye when carried out by an experienced team.

How Scars Heal and the Skill of the Surgeon

The surgeon makes the incision but the patient’s body heals by forming a scar. This is how skin heals following any form of injury. The degree of scarring will depend on the patient’s unique individual healing characteristics. In general, young men who have very elastic skin will have a very slightly wider scar than older individuals where there is less elastic tissue and the scar tends to be finer.

Some patients will be prone to a type of scarring commonly known as keloid scarring. In these patients the skin heals by forming a raised scar which is noticeable. Therefore, for patients that have a history of keloid scarring hair transplantation is usually not recommended.

The degree of scarring will also be affected by the skill and expertise of the surgeon and their team when carrying out your procedure. When someone poorly qualified carries out a hair transplant, often the patient can be left with unacceptable scarring in the donor and even the recipient areas.

When the procedure is carried out by a skilled team and the patient heals normally, then only a fine pale linear scar, in FUT procedures, or small pale circular scars, for FUE procedures should be left. These should be completely covered by the patient’s own hair.

Bryan Robson Hair Transplant – Update After 12 Months

Bryan Robson returned to HRBR, Dublin recently for a check-up following his hair transplant which was carried out in June of 2016. Bryan reported that he is absolutely delighted with the outcome of the procedure and noted that people have been making comments on how well and youthful he looks without actually noticing he has more hair. The change in his appearance is very subtle which is exactly what Bryan wanted to achieve with his hair transplant at HRBR. His hairdresser for 30 years made a comment that he has never seen his hair looking so well and was very complimentary about the outcome of his FUT hair transplant procedure.

Bryan Robson is a Global Ambassador for Manchester United. He mentioned that his hair transplant has made a big difference to him when he is meeting and greeting people as part of his role. He is heading to the US shortly with the Manchester United team and he said his hair transplant is so good that it has given him renewed confidence when dealing with the younger members of the team.

Bryan does not need to take any medication to maintain the transplanted hair and has decided not to take any oral medicines to help slow down his hair loss in the non-transplanted hair. He is, however, using Regaine foam which has brought about a significant improvement in the crown area. No grafts were transplanted into the crown region but Bryan is planning to have a further surgery in that area in the autumn to complete the job.

Overall Bryan is absolutely delighted that he went ahead with the procedure which was a big decision for him at the time. Scarring after Hair Transplant Surgery

This patient underwent two FUT hair transplants at HRBR. Our skilled hair transplant surgeons were able to remove the original linear scar during the second surgery and leave the patient with a single linear scar measuring just 1mm in width.

It’s important to remember that scar free surgery is not possible, regardless of whether you choose the FUT or FUE method of hair transplant surgery.

The Importance of Hairline Design in Hair Loss Treatment

Hair Transplant Design

A hair transplant is a permanent procedure; therefore it is vital that it looks natural. The key to a natural looking hair transplant is good hairline design. Careful design and planning is an essential part of the hair transplant process at HRBR. It is this careful planning and consultation with each patient that ensures that a hair transplant will look natural not only today but also into the future.

The hairline – where the person’s forehead meets their hair – is the part of the hair that is most noticeable, both to ourselves when looking in the mirror and by other people when out in public. If the hairline does not look natural, then the hair transplant will be noticed. A hair transplant should be seen, but never noticed. Any form of hair transplantation can result in poor hairlines if the ‘surgeon’ lacks an understanding of how a natural-looking hairline should be created.

Why Future Proofing the Design is Important.

Failing to consider future hair loss at the design stage can often leave patients with an unnatural and inappropriate look that becomes only more pronounced and more obvious as time passes. A patient suffering from pattern hair loss will continue to lose their non-transplanted hair throughout their lifetime. That’s a crucial factor to consider when it comes to designing a natural and undetectable hair transplant. This principle is at the core of our design approach. Feathering zone: Small peaks and a feathering of hairs using smaller follicular units will be placed at the front it to give the hairline a completely natural appearance.

Direction when planting: Consideration is also given to the size and colour of the hair grafts being transplanted, as well as the angle, depth and spacing of the transplanted hairs to ensure they seamlessly blend into their new location.

Anton Du Beke recently had a hair transplant at HRBR. We spoke to him about the experience and what led him to decide to have the procedure. Anton is a famous British ballroom dancer and TV personality who is most known for his role as a professional dancer on BBC’s ‘Strictly Come Dancing’. He has appeared as a dancer on the show since its beginnings in 2004. Anton said that during the last few years, he noticed his hairline was beginning to get thinner. It was after a conversation with his friend, and HRBR patient, James Nesbitt, that Anton decided to contact HRBR, one of the world’s leading hair restoration clinics, based in Dublin, Ireland.

“I was looking at photos of myself and images of myself on television and I noticed my hair loss more. This led me to start trying to cover it with make-up and to make sure I didn’t stand under certain lights. Basically, I didn’t like it very much. I knew it wasn’t going to improve so that’s when I started to think about having a hair transplant.”

Riyadh Hair Restoration

Riyadh Khalaf, YouTube star and presenter of the BBC series ‘Queer Britain’, is currently receiving treatment for his hair loss at HRBR, Hair Restoration Blackrock. Riyadh noticed his hair loss in his early twenties, particularly around his temples. As he says himself – he was very concerned by the discovery because his hair means the world to him. Riyadh took a proactive approach and arranged a consultation with the doctors at HRBR to discuss what we could do to treat his hair loss.

Our hair restoration doctors decided that the most suitable treatment plan for Riyadh’s hair loss was medication. He started his treatment, two FDA approved medications including Minoxidil in the form of Regaine foam. Sam also shared that he is sometimes teased by his colleagues about his hair loss, something else that is not unusual in patients experiencing hair loss. This is a good option for patients who wish to spread the cost of their hair transplant out over time. Several HRBR patients have done this. For example, patients have an initial hair transplant of 1,000 grafts transplanted, and as the cost of a hair transplant at HRBR is calculated on a per graft basis, there is no additional cost to undergoing the procedure in stages.

Having a hair transplant is a big decision for a number of reasons.  The cost of a high quality hair transplant can be a significant outlay for many people and understandably so. A bad hair transplant can have major long-term detrimental effects on the patient, both psychological and physical. Despite the claims of some clinics, there is no such thing as a non-surgical method of carrying out a hair transplant. A hair transplant is an elective surgery. With this in mind, what are the important things to consider before having a hair transplant?

  1. Do you need a hair transplant?

Nobody needs a hair transplant, it is elective surgery. If your hair loss is causing you concern, speak to an expert. They will talk you through your treatment options.

  1. The cause of your hair loss

There are many causes of hair loss but, in the overwhelming majority of men experiencing hair loss, the cause is hereditary Androgenetic Alopecia, more commonly known as “male pattern baldness”. Hair transplant surgery is a permanent treatment for Androgenetic Alopecia.

  1. Your age

It is important that the hair loss pattern is established before surgery is undertaken. Therefore, young men under 25 are often offered medical treatment initially, which usually proves very effective.

The second factor determined by a patient’s age is how it influences the hair transplant design. A patient suffering from pattern hair loss will continue to lose their non-transplanted hair throughout their lifetime. That’s a crucial factor to consider when it comes to designing a natural and undetectable hair transplant.

  1. Alternatives to hair transplant surgery

Medical treatment is available for hair loss. There are two US Food and Drug Administration (FDA) approved medications which have proven effective as a hair loss treatment in more than 80% of male users with hair loss.

This treatment is effective but not permanent. When you stop taking the medication, your hair loss will resume.

The cost of carrying out a quality hair transplant is considerable, as the process requires a team of highly skilled medical staff and technicians. The difference between FUT and FUE is essentially the way the donor hair being transplanted is removed from the patient’s scalp. Our patients ask us lots of questions prior to going ahead with a hair transplant at HRBR. That’s something that we actively encourage. One of the most frequent questions that our patients ask is ‘how will I look immediately after surgery?’ It’s a good question. How patients look after a hair transplant at HRBR

In our years of experience we have never had a patient come out of surgery having experienced this level of trauma to their scalp. The high tech follicular unit extraction (FUE) techniques that we use to remove hair from the donor area, combined with the skill of the surgical team who transplant those follicles to form the new hairline mean that our patients do not experience such an extreme reaction.

How painful is hair transplant surgery?

At HRBR our procedures are done under local anaesthetic and our patients usually describe mild discomfort at worst. Half of our patients do not require any pain relievers after surgery, and the others take over the counter medication for pain relief for a few days. Does a Hair Transplant Hurt?

One question that we are regularly asked by our patients is does a hair transplant hurt? The majority of HRBR patients are more than comfortable both during and after their hair transplant surgery but occasionally a patient says he / she had some discomfort or mild pain. A hair transplant procedure is carried out under local anaesthetic and over the counter pain medication is available for use afterwards should they be required. Many patients find that they do not need to take any pain killers at all after their hair transplant surgery.

Hair transplant surgery is an extremely safe minor surgical operation which requires only local anaesthetic. Patients have reported that the process of administering the local anaesthetic can feel initially uncomfortable, but this does not last long. At HRBR, we have developed techniques to reduce this discomfort, and most patients tolerate this brief period very well.
The donor area (the back or sides of the scalp) as well as the recipient area (the bald or thinning area) are made numb with the local anaesthetic which alleviates any pain during the hair transplant treatment itself.


How long does pain last after a hair transplant?

Once the hair transplant procedure is over patients can expect to have some redness on their scalp where the hair was transplanted, and this area may feel a little tender for a few days. Over the counter pain medication can be used to manage this should it be required. The pain from the procedure is minimal and the benefits of the surgery far outweigh any mild discomfort that the patients may experience.

The hair plugs inserted at this London based clinic left the patient with bad scarring at the back of his head and visible and unnatural plug grafts towards the front of the hairline. Our experienced and qualified surgical team took the time to assess the situation and developed a plan to remove the hair plugs and repair the hairline with hair transplant surgery.

As part of this hair transplant repair the team at HRBR initially removed some of the hair plugs in order to best prepare the patient’s hairline for the transplant. We then allowed the area time to heal in advance of the hair transplant.

Shampoo for Thinning Hair – Can it Help?

Many patients ask the doctors at HRBR for advice regarding shampoos and conditioners, either before they have a transplant or following their procedure. It is not a surprising question when an internet search for “Hair Loss Shampoo” returns over 1.4 million results. So, can shampoo for thinning hair help with hair loss?

A hair shampoo is basically a detergent that is used to clean the scalp and hair fibres and the majority of hair shampoos do nothing else.  Exaggerated claims of stopping or slowing down hair loss, for whatever reason, are not correct.

Some shampoos do have an ability to thicken the hair temporarily between shampoos however once washed out, the results disappear. With regards to a hair conditioner, we recommend that patients use a good quality one as these are the products that make a difference to the quality of the hair but unfortunately, they do not change the quantity of hair or have any effect on hair loss.

Our advice to our patients is not to over use hair shampoos as they do not bring about any medical benefit to thinning hair. There is no shampoo for thinning hair of which we are aware that has been scientifically proven to make hair grow stronger or to stop the progression of male or female pattern hair loss.

Recently a company who produce a caffeinated ‘hair loss shampoo’ was making outrageous claims about the increase in hair growth with their product. The Advertising Standards Authority have made them withdraw the advertising.

The reality is, unfortunately, there is no cure for male and female pattern hair loss at present. The only products that have been scientifically proven to make a difference to the progression of male and female pattern hair loss is Minoxidil 5% and for men the 5 alpha reductase inhibitor prescription medication which has been approved by the FDA.

The one exception to this is Ketoconazole shampoo “Nizoral”, limited clinical studies suggest Ketoconazole shampoo used either alone or in combination with other treatments may be useful in reducing hair loss by blocking the androgen receptor. As such, the doctors at HRBR have recommended that our patients use Nizoral shampoo as part of their treatment plan in some cases.

  • Ask to see the results of their work?
  • What will you look like immediately after the procedure? You should not leave your hair transplant procedure with bandages. For more details, see our blog on what to expect to look like immediately after a procedure.
  • What meetings have you had with the team in advance of your procedure? At HRBR we will meet with our patients twice in consultation in preparation for their procedure as the planning of a transplant is a crucial step in the process.
  • Has the clinic given enough thought to the design of your transplant? A poorly designed transplant can sometimes look fine now but as the patient loses more non transplanted hair it can start to look unnatural in years to come.
  • What about post-operative care? Hair transplantation is a surgical procedure and like any surgical procedure the care in the post-operative period can be just as important as the procedure itself.
  • Where will the procedure be carried out? Is it a clean environment? Clinics with poor hygiene can result in difficult post-operative infections that can jeopardise the newly transplanted hairs and can be difficult to treat.
  • We advise all of our patients to take their time in making a decision in relation to hair transplantation surgery. Visit as many clinics as possible and ensure you have trust in the team that will be looking after you.

Dr Maurice Collins Lectures as World’s Top FUE Surgeons gather in Lisbon

Medical director at HRBR, Dr. Maurice Collins, has just returned from lecturing in Lisbon at the World FUE Institute Meeting. With nearly 20 years of experience in hair transplantation, Dr Collins is internationally renowned as one of the world’s top FUE surgeons. He was asked to lecture on the FUE hair transplant procedure at the conference. He is considered a world expert on the selection of the donor area for FUE hair transplantation and also on how to maximise the total graft retrieval.  The HRBR team’s expertise in maximising the number of grafts from the donor area is one of the many reasons people travel from all over the world for FUE hair transplants at the clinic.

In addition to presenting at the World FUE Institute conference, Dr Collins was also asked to take part in the group discussions on difficult cases, an area of interest to Dr Collins and the team at HRBR. On the basis of his reputation as one of the top hair transplant surgeons in the world, Dr. Collins is one of the founding members of the World FUE Institute .

The FUE hair transplant area is rapidly expanding, with clinics opening daily around the world offering the procedure. However, with little or no regulation in the area many clinics open and close within a year. In many countries, including the UK and Ireland, there is no legal requirement that the doctors offering these services have any surgical experience. As a result, many patients are left with poor outcomes. Dr. Collins demonstrated some of these cases at the conference where patients suffered significant trauma to the donor region. Thankfully the expert team of surgeons at HRBR were able to assist most of these patients in achieving a better outcome.

As one of the world’s top hair transplant surgeons, Dr. Collins will continue to share his expertise with his peers. He will be lecturing in Brazil in August at the Brazilian Society of Hair Restoration Surgery on the FUE procedure and the advantages that can be obtained for the younger hair loss patient.  He will also present at the 1st International Aesthetic Surgery Meeting in Zurich this summer.
